TIME LIMITED CONTACT SCHEME 
This service aims to prevent changing times escalating into crisis situations for older people.
Examples of Changing Times would be:-
- When an older person loses a partner, or any close family bereavement
- When an older person comes out of hospital and has new disabilities to cope with
- Retirement
- Moving house to a different area
- Changes in financial situation
- The older person becomes a Carer
- Situations where someone has been caring for someone else and that person has either died or has moved into care.
The service will:
- help to prevent loneliness and depression
- Encourage independence
- Encourage activity and involvement
Referrals can be taken from individuals, Social Work Professionals and Health Professionals. Support will be provided for a maximum of 6 weeks to help the older person adapt to changes in their personal lives and provide some practical support, such as:-
- Phoning the person for a general chat, or to make specific reminders
- Accompanying someone to a hospital appointment
- Shopping with the person, going with them to a community facility, and helping them with other practical tasks
- Listening, providing information and encouragement
